This form is designed to be printed off, completed and returned to the surgery.
If you are off work sick for seven days or fewer, your employer should not ask you for a doctor's certificate. Instead, you should complete a self certificate.
Many employers have their own self-certification forms. Alternatively, they may use an SC2 form from HM Revenue & Customs Employee's Statement of Sickness.
You can complete an electronic version here.
Or print a paper version by clicking here.
For longer than seven days of sickness absence, you will require a doctor's certificate. Please contact the surgery regarding this.
